Pulmonary lipiodol embolism after transcatheter arterial chemoembolization for hepatocellular carcinoma

چکیده
Transcatheter arterial chemoembolization (TACE) has become an effective alternative treatment strategy for patients with inoperable hepatocellular carcinoma (HCC). Although TACE is relatively safe, pulmonary lipiodol embolism is a rare and potentially fatal complication which is often not anticipated by physicians. About 14 patients with pulmonary lipiodol embolism progressing to respiratory distress were reported in previous literature. Here, we report a patient with HCC who survived a severe pulmonary lipiodol embolism requiring mechanical ventilatory support and uneventfully received the second course of TACE. In this article, we sum previous literature and have discussed the possible pathophysiological mechanism underlying this complication and risk factors for its development.

Cerebral lipiodol embolism after transcatheter arterial chemoembolization of hepatocellular carcinoma.
A case of cerebral lipiodol embolism after transcatheter arterial chemoembolization (TACE) of hepatocellular carcinoma is presented. A 76-year-old man underwent TACE for advanced hepatocellular carcinoma. Immediately after chemoembolization, his level of consciousness deteriorated. Computed tomography revealed deposition of iodized oil in the cerebral cortex, basal ganglia, and thalami. Magneti...

متن کاملA rare complication after transarterial chemoembolization for hepatocellular carcinoma.
An 80-year-old woman was diagnosed with hepatocellular carcinoma and underwent transarterial chemoembolization (TACE). The procedure went smoothly without any complications. However, she presented with a dry cough 3 months after the intervention. On physical examination, she did not appear cyanotic. Chest auscultation revealed a minimal crackle sound in the perihilar lung fields; other respirat...
متن کاملPulmonary complications of transcatheter arterial chemoembolization for hepatocellular carcinoma
Transarterial chemoembolization (TACE) is an effective palliative intervention that is widely accepted for the management of hepatocellular carcinoma (HCC). Post-TACE pulmonary complications resulting in acute lung injury (ALI) or acute respiratory distress syndrome (ARDS) are rare events. Pulmonary complications after TACE are thought to be related to chemical injury subsequent to the migratio...
متن کاملAssessment of Lipiodol Deposition and Residual Cancer for Hepatocellular Carcinoma After Transcatheter Arterial Chemoembolization via Iodine-Based Material Decomposition Images with Spectral Computed Tomography Imaging: A Preliminary Study
BACKGROUND It is critical to follow up hepatocellular carcinoma (HCC) after transcatheter arterial chemoembolization (TACE) in clinical practice. Computed tomography (CT) is used to assess lipiodol deposition, whereas it is difficult to assess hypovascular residual cancer masked by lipiodol. In contrast, magnetic resonance imaging (MRI) is superior to CT in showing residual cancer, but cannot d...
